How to Replace Plates

My vehicle plates were lost, stolen or destroyed. What can I do?

You must surrender the vehicle registration to our Private Service Bureau. 


If you lose, have stolen, or if your vehicle plates are destroyed, certain steps must be taken:


  1. Notify the police and request them to complete a police report. This report is necessary to surrender the vehicle registration to the DMV.
  2. If the incident occurs in New York State, obtain a 'Certification of Lost License, Permit or Plates' (MV-78B) from a NY State police agency. If it happens outside of New York State, obtain a report from the police agency in that state.
  3. If the police agency refuses to provide a police report (MV-78B), you must complete a 'Certification of Lost License, Permit or Plates' (MV-1441.3) form, including the date of your request and the denying police agency's information.
  4. If only one plate is lost, stolen, or destroyed, you can surrender the remaining plate at your county's DMV office without requiring a police report. However, to replace the plates without a fee, you'll need to submit a police report or an MV-78B indicating the plate was stolen or destroyed due to a crime.
  5. Payment for Fees.

How to Replace Plates

You may request a new set of plates at a our Private Service Bureau Office.  Bring these items with

  • The original plate(s) or the MV-78B/police report/statemen
  • A completed Vehicle Registration/Title Application (All paperwork will be handled at our office to alleviate any stress.)
  • proof of insurance 
  • proof of your identity 
  • Payment for fees.
